Terry Pody, MSN, RN, NE-BC, of Fox Chase Cancer Center, explains how nurses can ensure that a patient goes to the right point of care after they have been discharged.

Many patients are eager to go right home upon their discharge. However, they are not always strong or well enough. The post-discharge phone call is essential for the nurse to be able to determine whether a patient may need home care or perhaps even a rehabilitation center. Ensuring that a patient goes to the appropriate level of care after their discharge is the best way for nurses to prevent readmissions.
