Terry Pody, MSN, RN, NE-BC, of Fox Chase Cancer Center, explains how nurses can ensure that a patient goes to the right point of care after they have been discharged.

The post-discharge phone call is essential for the nurse to be able to determine whether a patient may need home care or perhaps even a rehabilitation center. Ensuring that a patient goes to the appropriate level of care after their discharge is the best way for nurses to prevent readmissions.
