
Choosing the Optimal ARPI for Older Patients With Prostate Cancer

For oncology nurses, the challenge lies in balancing aggressive treatment goals with the individual health profiles of an aging population. Sandy Srinivas, MD, medical oncologist and professor of medicine (oncology) and of urology, at Stanford Health Care, emphasizes that the journey toward optimal outcomes begins with a rigorous assessment of chemo-fitness.
By identifying patients who can robustly handle the initial intensive phases of treatment, clinicians can better ensure the successful delivery of all six cycles of chemotherapy. This foundational phase is supported by vigilant symptom management, utilizing growth factors and blood count monitoring to bridge the gap toward the transition to androgen deprivation therapy (ADT) and androgen receptor pathway inhibitor (ARPI) doublets.
As the oncology community moves away from a "one-size-fits-all" approach, the selection of specific ARPIs is becoming increasingly data-driven, even in the absence of head-to-head randomized trials. Srinivas highlights a growing body of evidence suggesting that the choice of an ARPI must be tailored to the patient’s preexisting conditions, particularly in older adults.
For instance, while abiraterone acetate (Zytiga) remains a cornerstone of therapy, its association with an increased risk of hypertension and atrial fibrillation may make it a less-than-ideal candidate for patients with underlying cardiovascular concerns. In these clinical scenarios, transitioning to one of the "-amide" agents as a doublet partner can offer a more favorable safety profile, allowing for sustained treatment efficacy without compromising the patient’s quality of life or cardiovascular stability.
Effective navigation of these treatment pathways requires a collaborative effort to monitor for late-onset side effects and ensure medication adherence. Nursing teams play a pivotal role in this process, serving as the first line of defense in identifying the subtle shifts in blood pressure or cardiac rhythm that might necessitate a shift in therapy. By understanding the emerging data surrounding ARPI selection, oncology nurses can provide more personalized education and advocacy for their patients.

The primary consideration in optimizing treatment is patient selection, ensuring we choose the most appropriate therapy for each individual. A critical factor in this decision-making process is chemo-fitness, which determines who is truly a candidate for intensive regimens. Once a patient begins those six cycles of chemotherapy, the focus shifts to proactive management. This includes monitoring blood counts and utilizing growth factors to manage symptoms effectively, with the ultimate goal of ensuring all six cycles are successfully delivered before the patient transitions to the ADT and ARPI doublet.
Beyond chemotherapy, we are gaining a deeper understanding of how to select the most appropriate ARPI for a given patient. While there are currently no head-to-head randomized trials comparing one ARPI directly against another, emerging data provides valuable guidance. For instance, in our older patient population, abiraterone may not be the optimal choice due to an increased risk of hypertension and atrial fibrillation. In these cases, selecting one of the "-amide" agents as part of the doublet therapy may be a more appropriate and safer clinical approach.
